Vulnerability Based Triage (V.B.T.)

Vulnerability Based Triage is a community service offered by Methodist Hospital of Chicago. V.B.T. provides services in the home to persons 21 and older with medical, behavioral, or cognitive health needs. Services are provided at no cost and include a visit from a team of medical and mental health professionals.  

This team will work with the vulnerable person and their family members or other concerned individuals to identify the problem and recommend resources. Referrals can range from homemaker referrals to hospitalization, from nursing home placement to short-term case management. This is not an emergency line; if an emergency exists, always dial 911.
