The Community Health Needs Assessment is a systematic, data-driven approach to determining the health status, behaviors and needs of residents in the primary service area of Thorek Memorial Hospital Andersonville.

The Implementation Planning Process

After considering the findings of the CHNA in the context of current community-focused activities and resources, Thorek has decided to address the following health priority areas (in no particular order):

  1. Access to Healthcare Services
  2. Cancer Care
  3. Mental Health
  4. Oral Health
